Need a refill on your pet’s medication? Submit your request using the form below and our team will process it as quickly as possible. Please allow adequate time for processing—some prescriptions may require an updated exam or veterinarian approval before a refill can be authorized.
Before You Complete the Form
To help us process your request promptly, have the following information handy before you begin:
- Your name, phone number, and email address
- Your pet’s name, species, and breed
- The name of the medication you’re requesting
- The dosage and frequency at which your pet is currently prescribed
- The name of the prescribing veterinarian, if different from your pet’s regular veterinarian
- The date of your pet’s most recent exam, if known
Please note that prescription refills cannot be guaranteed without an active Veterinarian-Client-Patient Relationship (VCPR). If your pet is overdue for their annual exam, our team may reach out to schedule a visit before the refill can be processed.
