If your pet isn’t feeling well, we want to be ready for them the moment you arrive. Completing this form before your appointment helps our team understand your pet’s symptoms, prepare appropriately, and make the most of your visit.
Before You Complete the Form
To help our team prepare for your pet’s sick visit, have the following information handy before you begin:
- Your name, phone number, and email address
- Your pet’s name, species, breed, age, and sex
- A description of your pet’s current symptoms and when they first began
- Any changes in your pet’s eating, drinking, energy level, or bathroom habits
- Your pet’s current medications, supplements, or known allergies
- Any recent changes in your pet’s environment, diet, or routine that may be relevant
- Whether your pet has experienced these symptoms before and, if so, how they were treated
- Any potential toxin exposure or foreign objects your pet may have ingested
The more clearly you can describe what you’re observing at home, the better equipped our team will be to help your pet right away.
