Before You Complete the Form
To help us locate and process your records request accurately, have the following information handy before you begin:
- Your name, phone number, and email address
- Your pet’s full name, species, and breed
- The specific records you’re requesting (e.g., full medical history, vaccination records, lab results, surgical reports)
- The date range of records requested, if applicable
- The name, address, and contact information of the receiving provider or facility, if records are being transferred to another clinic
- Your preferred delivery method for the records (e.g., email, fax, pick-up)
If you’re requesting records for more than one pet, please submit a separate form for each animal to ensure accuracy.
